Microwave thermotherapy of BPH with CoreTherm®

- Brief information for the treating/referring physician

CoreTherm® for whom?

  • Symptomatic BPH
  • Prostate size > 30 g
  • Prostate length ≥ 35 mm

Investigations

  • Standard investigations for BPH
  • Medical history
  • Symptom score (IPSS-score incl. bother score)
  • Voiding diary
  • Flow measurement
  • Physical investigation, incl. rectal palpation
  • Transrectal ultrasound (TRUS)
  • Urinalysis, incl. hematuria and bacteriuria tests
  • S-creatinine
  • Prostate specific antigen (PSA), recommended for patients under 70 years of age

Optional investigations

  • Urodynamics
  • Urethrocystoscopy

Contra-indications

  • Urethral stricture
  • Patients with penile or urinary sphincter implants
  • Previous radiation of pelvic region

Inform the patient

  • that CoreTherm® is a policlinical treatment
  • that the treatment is performed with microwaves via a catheter that is inserted into the urethra
  • that the treatment will take approximately 10-30 minutes with local anesthesia
  • that he might experience a (uncomfortable) sensation of heat and an urge to urinate during treatment

That after CoreTherm®

  • a urinary catheter will be needed up to three weeks after treatment
  • small pieces of dead tissue might be discharged with the urine and a small amount of hematuria could occur
  • the urge to urinate may occur (after the catheter has been removed), but this will gradually diminish
  • there is a risk of retrograde ejaculation
  • over 80% of patients experience significant improvement

Other

  • a temporary increased level of PSA can be found after CoreTherm
  • hand out a patient brochure
